Select a time and date that suits you and your kid. Once you have selected this, press ‘Continue’ to proceed.

Please fill out the form with the participant’s name as well as your phone number and email address.

After you’ve filled out the form, please read and agree to the participant’s and parent’s code of conduct. Once you have read and agreed, please press ‘Complete Appointment’.
